The Student Registration and Enrollment Process

*First Day Exclusion Policy:
It is a State of Illinois requirement that all children entering Preschool, Kindergarten and 6th Grade submit a completed Certificate of Physical Health Examination Form (including documentation of all required immunizations). If a child’s required physical and immunizations are not submitted prior to the first day of school attendance, delay in enrollment WILL occur. All physicals and immunizations forms must be documented on the Illinois approved form and be reviewed and certified by the school nurse prior to the student being enrolled and attending school. Prior to submitting your child’s completed medical form(s), parents are advised to make a photocopy for their records, as copies will not be made at the schools or District Office.
